As part of our mission to improve the way in which we deliver care and improve our services, Chatfield Medical Centre has reviewed patient feedback and we have launched our new online consultation service. This simplifies the process to submit medical and admin requests, and patients are no longer required to create an account.
All requests coming into the practice are dealt with via the online form. If you are unable to use a smartphone or computer, or do not have access to the internet, one of our Patient Services Advisors will be available to go through the form with you either over the phone or at the front desk. This will ensure that all important details are captured in order for us to triage your request.”
